This form is to be completed by each club wishing to host a summer mini-football. After completion it must be sent

to Jon Larsen, 4 Min-yr-Awel, Pen-y-fai, Bridgend CF31 4GJ or emailed to jg_larsen@yahoo.co.uk

Following authorisation by the mini-football management committee, your requested date will be placed in the diary

of fixtures. All dates will be allocated on a "first come first served basis". If for any reason your application is rejected

you will be informed immediately, with the reason for rejection, and asked to resubmit a modified application
